天后宫注册的你，结婚注册表格可以用上网下载的吗？

如题，如果要在天后宫注册，注册表格可以从JPN网站下载？还是一定要到该部门拿表格？ |

pc cannot type in chinese..
if im not mistaken, u need go to JPN n get the JPN officer signature on the form then only can submit to Thean Hou Temple |
