Date: 23rd August 2009
Venue: Exam Hall
Time: 8.30am until 1 pm
2.00pm until 5 pm
*Faculty of Business Law (FBL)
Date: 24th August 2009
Venue: Exam hall
Time: 8.30am until 1 pm
2.00pm until 5 pm
*Centre for foundation studies & extension education (FOSEE)
Date: 25th August 2009
Venue: Exam Hall.
Time: 8.30am until 1 pm
2.00pm until 5 pm
*Faculty of Engineering and Technology (FET)
*Faculty of Information Science & Technology (FIST)
Date: 26th August 2009
Venue: Main Hall.
Time: 8.30am until 1 pm
2.00pm until 5 pm
*Centre for Diploma Programmes (CDP)
Students are required to bring along:-
1. BLACK BALL PEN ONLY
2. Ruler
3. Glue
4. A copy of your IC (certified true copy)
5. A copy of your parent’s IC (certified true copy)
6. A copy of your CIMB account book (front page only, also certified true copy
7. Duty stamp (setem hasil) - 2 pieces, worth RM10.00 each.
8. A copy of death certificate/ divorce agreement/ letter from oath commissioner if one of the parent/parents is passed away or divorced or divorced WITHOUT ANY WRITTEN DOCUMENTS.
9. Full transcript and verification student status letter if you are in Beta or Gamma year.(3 working days for request)
10. Income verification form (certified by oath commissioner)
11. 2 copies of Online Agreement.
NOTE: =
1. Please be punctual for this event. Late comers will not be entertained.
2. Students are reminded to wear student ID/ temporary student ID. Failing to do so, you will not be allowed.
3. Attire: Formal, students are to dress decently and neatly.
4. Caps, slippers OR non - strapped sandals are not allowed.

请问ptptn form里面的saksi是要给谁签的？是要ptptn day当天给ptptn的人员签，还是可以自己拿去给有头衔如dato的人签？ |
